CARECONNECT HEALTH INFORMATION EXCHANGE AUTHORIZED USER AGREEMENT

IMPORTANT-READ CAREFULLY: Our Health Information Exchange (HIE) application provides secure, encrypted access to confidential patient information. In accordance with federal HIPAA regulations, unauthorized disclosure or misuse of patient data is strictly prohibited and may subject liable parties and/or institutions to civil and criminal penalties.

By using the HIE, you acknowledge your agreement to the Terms of Use.

Please note that there has been a change to our Privacy Policy stating that CareConnect HIE users should not print records from the HIE to provide a patient access to their medical record.

For password resets or technical support issues, please call the Atrium Health CareConnect Service Center at 1-888-724-0459, and press Option 3.
